I am resending the eblast sent out on December 22 (below) to remind families whose children are returning to school, that every student in blended learning needs to have signed the new COVID consent form completed before they can return to the school building. Please see details below.

Dear Families,
For students to attend in-person learning when schools re-opens, an updated consent form for weekly in-school COVID-19 testing must be on file.
There are two options available to consent to COVID-19 testing in schools.

Print the updated form at this link, fill out and return it with your child on their first cohort day. This form will be collected at arrival and will be used, along with the health screener as part of our arrival procedure. If you are unable to print this form out at home, we will have additional copies for you to complete on your child's first day of in-person learning
- Log into
mystudent.nyc. Make sure you click the box agreeing to the privacy terms just above the sign in button.
- See "Manage Account" at the top and click on it.
- Click "Consent Forms"
- Click "Covid 19 Testing"
- Follow directions and click "Submit"
If you do not already have a NYCSA account, please email Ms. Gonzalez at
sgonzalez@mail.wagner167.org for your creation code. If you have an account creation code, you can create a full account and then provide consent. If you do not have an account creation code, you can still provide your consent by clicking “Manage Consent” under “COVID-19 Testing” and filling out your child’s information to provide consent.
